The incumbent is responsible for the clinical aspect of patient care including phone and walk-in triage. Assists with procedures, patient education, outpatient and post hospital discharge follow-up including coordinating home care, hospice, equipment, physical and occupational therapy. The nurse incorporates the psycho-motor and age appropriate cognitive skills of the patient/family/significant other utilizing knowledge of patient assessment, planning, intervention and evaluation.

Required Qualifications:
Ability to establish and maintain effective working relationships across the Health System
Ability to maintain a work pace appropriate to the workload
Must demonstrate customer service skills appropriate to the job
All external candidates must have a Bachelor of Science in Nursing
Current California RN license
Minimum of 6 months demonstrated nursing experience in an acute care or ambulatory care setting
Ability to read, write and communicate effectively in English
Ability to work variable shifts including evenings, nights, weekends, and holidays
Ability to maintain flexibility and work well in a constantly changing environment
Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
Licenses and Certifications:
Basic Life Support(Required)
Preferred Qualifications:
Understanding of applicable regulatory requirements
Cardiovascular clinical experience
Bilingual skills to communicate effectively with patients and families
