Overview Creative Support Alternatives, a San Diego State University Research Foundation project for 30 years, provides Supported Living Services, Independent Living Services and related services to adults with developmental disabilities, as well as others who benefit from individualized, person-driven supports, to maximize their full inclusion and participation in all aspects of home and community life.
The Community Support Facilitator, under the supervision of the Director(s) and Community Living Coordinator(s), provides direct support to people with developmental disabilities in their homes and in the community. Each position is individualized in order to support the needs and wants of the person receiving services.

Responsibilities DUTIES
80% or more of duties
Provide 1:1 support (support/instruction/guidance/etc) in the focus person's home, as well as in the surrounding neighborhood and community. Support may occasionally be provided to more than 1 person at a time when appropriate/relevant to the situation.
Provide personal support to the focus person which may include assisting the person to use the restroom, take a shower/bath, perform personal hygiene, wear weather appropriate clothing, etc.
Assist the person to perform household tasks and chores, which may include but not limited to cooking, cleaning, organizing, washing laundry, etc.
Assist the focus person to perform chores/tasks outside the home, which may include but not limited to grocery shopping, using public transportation, performing errands, participating in community events, etc.
Follow Individualized Support Plan, as well as policies and procedures to aid in the maintenance of the focus persons overall health and well-being.
20% or less of duties
Coordinate Community Living support services with any community services, such as Supported Employment, Day Program, or similar.
Initiate, coordinate, implement, and participate in person-centered planning processes in collaboration with the focus person and other support personnel.
Function as an 'anchor', facilitating the development/enhancement of the focus person's 'paid' and 'non-paid' circles of support.
Advocates with/for the focus person, securing necessary generic community resources and supporting his/her participation in the community activities and groups in collaboration with others in the focus person's network of support.
Utilize sound judgment and effective problem-solving skills to promote the safety and well-being of the focus person.
Assist the focus person and his/her network of support in identifying/developing/implementing meaningful Individualized Support Plans (ISPs).
Develop and utilize individualized adaptations, as needed.
Assist in 'orientating' new staff/support persons to the focus individual.
Participate in in-service trainings, workshops, monthly meetings, as requested.
Write/Submit required documentation.
Positively represent both the focus person and Creative Support Alternatives in the community.
Communicate effectively with focus person, family members, supervisors, SDSURF staff, coworkers and community members.
Provide overnight support as deemed necessary.
Other responsibilities may include assisting the focus person to interview in-home support workers, or other support personnel as deemed necessary by the Co-Directors and Coordinator(s).
Qualifications MINIMUM EDUCATION
High school diploma or GED equivalency
MINIMUM EXPERIENCE
One-year of experience functioning as a team member in group or work setting
