Home Hospice Physician – Seattle Eastside

EvergreenHealth is currently recruiting a part-time (.5 FTE) Home Hospice Physician to join our exceptional and growing Hospice Care Team at EvergreenHealth Medical Center in Kirkland, WA. Join a dynamic group of Hospice providers in a Seattle-area healthcare system widely recognized as a national leader in end-of-life care. EvergreenHealth home hospice program (average daily census of 500+ patients) with a freestanding inpatient hospice care center. 

Job Duties:

• Provides clinical care to Hospice patients in the community setting.
• Responds to calls regarding care of hospice patients and participates in hospice medical provider call rotation.
• Evaluates patient eligibility for hospice services upon referral and throughout patient’s hospice enrollment.
• Participates in interdisciplinary team and hospice medical provider meetings.
• Participates in and provides staff education and support regarding end-of-life care.
• Performs other duties as indicated.
• Strong preference for candidates who are specialty board-certified/board eligible in Hospice and Palliative Medicine and/or have completed Hospice Medical Director certification (HMDC).
