CATCH program: Collaborative Advocacy through Community Healthcare. This new collaborative connects three existing OHSU community-based programs (Interprofessional Care Access Network, Care Transitions Innovation, and New Directions) to enhance services for people with complex social and health needs in two underserved neighborhoods in the Portland metro area. HEARST grant funds has backed up this program to include admin support to re-coup money for billed services.
The CATCH PAS Specialist in Care Management, working as a member of the interdisciplinary team, provides assistance and support to the SoN and Care Management division by tracking data, monitoring billing queue, scheduling appointments and more. The PAS specialist supports theTransitional Care programthrough monitoring metrics, scheduling appointments for the care transition staff, ordering supplies for patients and attending meetings. This position functions under the direct supervision of the Healthcare Program Manager. Direct face to face interaction with patients and/or families may be required.
Transition Care/ CATCH:
Send out C-Train discharge patient email
Monitor billing queue/ resolve any patient case issues
Put together and send out C-Train packet for patients
Order supplies for the programs (patients):
Tracfones new and add ons
Manage the hotel voucher system for patients scheduling post hospital hotel stay
Manage city team vouchers
Schedule appointments for patients seeing care transitional staff
Support Billing for the CATCH- SoN monitor queue
Create supporting documentation for Transitional teams
Track patients enrolled in program and oversee discharge documentation
Create dashboard for monthly reporting on Transitional Care programs
Performs tasks as assigned
Six months of work experience in a medical office or hospital setting, including high volume direct patient contact OR on one year of work experience in a high volume direct public contact position.
Demonstrated excellent written and verbal communication skills.
Basic keyboarding skills.
Strong customer service.
Successful experience working with patients and their families.
Flexible, self-motivated, proficient at multitasking, accustomed to frequent interruptions, and comfortable with change.
Proficient with Excel data entry.
Ability to create simple Word documents.
Must be able to perform the essential functions of the position with or without accommodation.
Knowledgeable of medical terminology and patient discharge process.
